Terms & Conditions
At AMF Sport (PC0015517-X) (“Team Armada Martial Arts”), we have adopted policies, procedures, rules and regulations, terms and conditions (“Policies”) designed to provide for a safe, enjoyable and healthy use of Team Armada Martial Arts premises, classes and / or activities for members, personal training clients and guests (hereinafter referred to as “you”, “your” or the “Member”).
These Policies apply to your conduct within Team Armada Martial Arts premises. The Policies also apply to your conduct during Team Armada Martial Arts programs, training, workshops, events and / or other activities held off the premises involving Team Armada Martial Arts (collectively known as “Activities”).
The following Terms & Conditions govern the rights and obligations of Team Armada Martial Arts and its Members:
GROUP MEMBERSHIP AGREEMENT
By signing and agreeing to Team Armada Martial Arts’ Membership Agreement (hereinafter referred to as the “Agreement”), you understand that you have accepted and entered into this Agreement with Team Armada Martial Arts and have read, understood and agreed to all terms hereof.
You shall be at the age of 18 years and above when you enter into this Agreement. If you are under the age of 18, you represent that you have obtained the consent of your statutory representative(s) prior to entering into this Agreement.
Your personal information will be taken and collected by Team Armada Martial Arts for our records and identification purposes. You hereby agree that we may release any information about you as where required to do so by law.
1) Memberships are personal to the individual signed up and is non-shareable, non-assignable, non-refundable and non-transferable. Memberships also cannot be transferred into other services such as Personal Training and Small Group Training.
2) You are required to pay an initial new member setup fee in the sum specified, unless stated otherwise. These fees are non-refundable and are due immediately upon registration to become a member. You will be subjected to registration charges again if you have no active membership of more than 1 year.
3) Fees vary depending upon the type of membership package subscribed. Your fees are as specified in your initial consultation with the representative of Team Armada Martial Arts and are due in advance.
4) With a Team Armada Martial Arts membership, you will have unlimited access to the group classes according to the age limit. As such you are to abide to the booking, cancellation and conduct guidelines stated in the “Accounts & Policies” section below.
5) Additional benefits are available with Team Armada Martial Arts membership. You will be able to get 20% discount for merchandize purchases and 10% discount for early renewal 1 month prior to existing membership expiry. Upon successful referral signing for membership of six (6) months-term and twelve (12) months-term, Team Armada Martial Arts will present you with complimentary one (1) Session Personal Training.
6) Upon completion or expiry of the membership term, you have the option to renew your membership with the same package or opt for other packages. If you previously signed up for a membership under a promotion you will need to revert to the normal membership fees, unless there is another valid promotion available. The registration fee will be waived for renewals.
7) For membership of twelve (12) months-term, you may perform membership pause of 1 month block duration for up to three (3) times. For membership of six (6) months-term, you may perform membership pause of 1 month block duration for up to two (2) times. For membership of three (3) months-term, you may perform membership pause of 1 month block duration for only one (1) time. Membership pause fee of RM60 each time for 1 month block duration will be imposed to retain membership during this period and no back dated hold will be allowed. Early return during membership pause duration will not be refunded and membership will be reactivated immediately upon return.
8) Members may contact management directly at teamarmadapg@gmail.com should he / she decide to cancel their membership, stating the reason for said cancellation. However, there will be no refunds given for such eventuality.
9) For monthly payment plans, the first payment consist of new member setup fee, deposit of one month membership price and pro-rata first month membership price.
10) For monthly payment plans, payment must be made before the 5th of the month. Late payment charges of 10% of the monthly payment will be charged if the payment is not made by the 5th of the month.
11) For monthly payment plans, membership will be deemed as failure to comply to terms and conditions. Membership will be cancelled and deposit will not be refunded if payment is not made by 7th of the month.
12) You recognize that you will be involved in strenuous physical activity including, but not limited to, muscular strength and endurance training, cardiovascular conditioning training, and other various activities. You warrant and represent that you are in good physical condition and do not suffer from any known disability, disorder, or condition that could be triggered or worsened by participating in any Team Armada Martial Arts training program.
13) Team Armada Martial Arts reserves the right to terminate the membership for reasons that may include unruly or disruptive behavior, breach of this Membership Contract, and similar. There will be no refunds given under these conditions. The decision of Team Armada Martial Arts is final.
PERSONAL TRAINING & SMALL GROUP TRAINING AGREEMENT
We’re glad that you are joining our Personal Training program! Please read and review the below terms and conditions and kindly sign it at the bottom if you agree to them.
PAYMENT, EXTENSION & EXPIRATION
Packages must either be paid in full (cash/bank transfer/credit card/debit card) directly to Team Armada Martial Arts before the commencement of your first training session.
Kindly take note of your Personal Training and Small Group Training package validity period:
1 Session – Immediate
10 Sessions – 3 Months
20 Sessions – 6 Months
In any case which the purchased package will not be finished before the expiration date, all the remaining sessions will be forfeited. There will be no refunds given once payment has been made.
Package validity extension at the maximum of 1 month is subjected to charges of RM150 or renewal of package with equal or higher value. Validity extension can only be done once per package and the remaining sessions will be forfeited upon the new expiration date.
CANCELLATION POLICY
The time of sessions is to be agreed upon between the trainer and the member. You must arrive on time for the scheduled appointment. If you arrive late by any chance, do understand that your session will end at the originally scheduled time, unless other arrangement has been made previously with the trainer.
If you fail to attend your session, or cancel within 12 hours of the scheduled appointment, your session will be considered a completed session unless evidently proven that it’s a case of an emergency. Conversely, if the Trainer cancels on you within 12 hours, you will be given a replacement session at no extra charge unless evidently proven that it’s a case of an emergency.
Any tardiness of more than 15 minutes or absence without proper notification will result in forfeiting the session and no sessions or payment reimbursement will be granted. All training package sessions will start and end no more than allocated time of 1 hour.
Small group training sessions are deducted as a whole in the case where there is one or more absentee(s) for the originally scheduled appointment.
ACCOUNTS & POLICIES
PURCHASE OF PACKAGES
A Team Armada Martial Arts account must be created before any package can be purchased. Packages may be purchased online, on the phone or in-person. The expiration of the package will start immediately after the confirmation of purchase email has been sent to your account email. Payment can be made online or within our premise, via cash or card. If you have mistakenly purchased a new package and received a confirmation email, please contact Team Armada Martial Arts immediately so we can amend this accordingly.
RESERVATIONS
Bookings may be done on our online system. Last call for reservations are fifteen (15) minutes before the reserved class time. All attendees must be physically present within the premises at least 15 minutes before the commencement of the reserved class – if you are more than ten (10) minutes late, please inform the instructor beforehand and join in the class promptly.
WAITING LIST
Should a class be fully booked, you can still reserve a spot for a class but you will be added to the waitlist. You will be informed via email if you have been booked in to the class.
PERSONAL BELONGINGS
Any purchase of a membership or studio package from Team Armada Martial Arts acknowledges that Team Armada Martial Arts is in no way responsible for the safekeeping of your personal belongings when you are present in Team Armada Martial Arts premises. You assume all risk of loss for any of your personal belongings.
GENERAL
PERSONAL FITNESS DECLARATION
Before engaging in any fitness activities at Team Armada Martial Arts, we strongly advise that you obtain a medical clearance from a certified medical practitioner should you have any doubts about your physical capability to engage in any physical activities.
You are to abide with Team Armada Martial Arts’ rules, policies and instructions including warning notices when using our equipment and facilities. Please notify us immediately should you experience any illness or discomfort physically. Team Armada Martial Arts is not responsible in terms of medical requirements of our members. Participant understands that in case of injury, the only medical treatment Team Armada Martial Arts will provide is first aid.
LIMITATION OF LIABILITY
In consideration with the registration, Team Armada Martial Arts agrees to provide participant with training activities such as group class activities, personal training and small group training.
In exchange, participant understands that there is a risk of personal injury in the course of instructions and with this knowledge, agrees to assume the risk of any injury and damages to participant during the activities.
Specifically, participant agree to participating in physical exercise or training activities at their own risk and hold Team Armada Martial Arts, it’s associated companies, employees, agents, all other individuals and other participants connected with the activities harmless on account of death or due to injury, loss, damages, theft, causes of actions, claims, or complaints against them by or on a member’s behalf for any such injuries or claims aforesaid. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence. Participant further agrees to strictly obey instructions and observe safety rules.
Any guest of a member or a temporary visitor to Team Armada Martial Arts shall agree to abide by Team Armada Martial Arts’ rules and the same limitation of liabilities for a member shall apply.
Member hereby agrees that any pictures, audio, or visual recordings taken of him/her in connection with the activities can be used for publication, promotion, articles, shows and advertisement without additional consent and without compensation at this time or any other time.
